If you are a Louisiana resident living with a temporary or permanent disability you can apply for a Louisiana handicap parking permit through The Notary Shoppe. We offer handicap tags including hangtags or placards at any of our five south Louisiana offices. The Notary Shoppe works with the Louisiana OMV to facilitate handicap parking permits. As such all permits issued through The Notary Shoppe must be compliant with up to date requirements from the State of Louisiana.
How to Obtain Louisiana Handicap Parking Permits
Searching for how to get a handicap tag in Louisiana? Before you arrive there are several steps that must be completed in order to issue your handicap tags. Please ensure that this is completed before you arrive in order to ensure a smooth process when you visit us in person. Please bring with you a properly filled out Physician’s Certification of Mobility Impairment. This is a requirement in order to get a mobility impaired parking permit in Louisiana, and The Notary Shoppe will not be able to process your application without this form filled prior to your visit.
In addition to the Physician’s Certification of Mobility Impairment form, applicants must also present a valid Louisiana driver’s license. An Important Note On Handicap Tags
In the state of Louisiana, handicap hangtags are not issued permanently – you must renew your permit over time. The duration of your permit depends on the nature of your disability. Your service provider can talk you through how long your permit lasts during your visit. Depending on the nature of your disability you will need to renew your handicap license tag in Louisiana at different times.
For people with temporary disabilities, handicap tags must be renewed every single year. This will require a newly filled out form of Physician’s Certification of Mobility Impairment.
For people with permanent disabilities, you will be required to renew your handicap placard in Louisiana every four years. Permanent disabilities do not require a new Physician’s Certification of Mobility Impairment form. Even without the form you must renew your mobility impaired parking permit in Louisiana every four years.
Who Is Eligible for A Handicap Placard in Louisiana?
It’s important to talk to your doctor to see if a handicap tag is an option for you. Not everyone is able to get a handicap permit. In order to be eligible you must be considered “mobility impaired”. The following are the six possible conditions that would designate someone as mobility impaired:
- Cannot walk two hundred feet without stopping to rest.
- Cannot walk without the assistance of another person, walker, cane, crutches, braces, prosthetic device, or wheelchair.
- Is restricted by a lung disease to such an extent that the person’s forced (respiratory) expiratory volume for one second, when measured by spirometry, is less than one liter, or the arterial oxygen tension is less than sixty mm/hg on room air at rest.
- Uses portable oxygen.
- Has a cardiac condition to the extent that the person’s functional limitations are classified in severity as Class III or Class IV according to standards by the American Heart Association.
- Has a diagnosed disease or disorder, including a severe arthritic, neurological, or orthopedic impairment, which creates a severe mobility limitation.
Physician’s Certification of Mobility Impairment Form
A requirement for first-time handicap permits, or renewal for temporary disability is the Physician’s Certification of Mobility Impairment. This form MUST be filled out by the applicant’s doctor, or medical examiner. Without this form properly filled out by a doctor The Notary Shoppe is unable to issue handicap tags.
Your doctor will fill out and assure that you are mobility impaired to the level of requiring a handicap tag. They will mark if this is a permanent handicap or a temporary handicap.
You will not fill out this form yourself but you must be able to provide your car’s VIN number, and license plate number.

Please make sure to arrive a minimum of 30 minutes before our location closes in order to be served. All The Notary Shoppe locations are first-come-first-served and we may not be able to serve you at the very end of the day if there is already a line.
