Louisiana residents living with a temporary or permanent disability can apply for a disabled parking permit through the Notary Shoppe. We issue the handicap tags (hangtags or placards) at five convenient locations in Southeastern Louisiana.
What You Need to Know About Louisiana Disability Hang Tags
Before you obtain or renew your Louisiana handicap tags, please bring in this form filled out: Physician’s Certification of Mobility Impairment. This is a requirement and The Notary Shoppe will not be able to process your application without this form filled prior to your visit.
In addition to the form, you will also need a valid Louisiana driver’s license/state ID. The Notary Shoppe also is able to renew Louisiana drivers licenses.
Remember you will need to renew the handicap hangtags over time, they are not a permanent option at this time. Handicap Tags for temporary disabilities must be renewed each year and will require a new Physician’s Certification of Mobility Impairment. Hang Tags for Permanent Disabilities must be renewed every four years, but do not require a new certification.
